This study investigated the long-term effectiveness of tension-free vaginal tape obturator (TVT-O) as a treatment for urinary incontinence in women. Researchers suggested that TVT-O is a safe and highly effective option for the long-term treatment of this condition.
Some background
Stress urinary incontinence (SUI) affects 4 to 35% of an adult woman. It consists of the leakage of urine after a physical effort (stress) such as sneezing or coughing. There are a few treatment options available. Even though treatments such as TVT-O have been shown to be safe and effective, it is not included in the guidelines. TVT-O consists of the insertion of a special tape under the urethra (urinary tube). The tape is inserted through a cut inside the vagina and a cut in the obturator muscle (pelvis area). This will help to support the urethra in place, preventing leakage.
Prior studies suggested that methods like TVT-O are associated with improved outcomes. However, the long-term effectiveness and safety of TVT-O are not clear.
Methods & findings
This study included information about 168 women. These patients had SUI and received treatment with TVT-O. They were followed up for 13 years.
At 13 years after surgery, 150 of 157 (95%) considered themselves cured. Patients who underwent previous anti-incontinence procedures were at 5.8 times more risk of a failed TVT-O. Obese patients were at 1.6 times more at risk of an unsuccessful TVT-O. Tape exposure was found in 4 patients after the 10-year follow-up.

This study concluded that TVT-O is an effective and safe method to treat stress urinary incontinence in women long-term.
