This study investigated factors that might affect male fertility. They found that a healthy diet and weight are important for male fertility. 

Some background

Male infertility (MI) is an increasing problem worldwide. In many cases the cause of infertility is unclear. Semen quality is determined by measuring a number of parameters. These include semen volume and sperm concentration, count, motility, and morphology (shape, SPM)

The role of environmental and lifestyle factors in fertility problems is unclear. Some studies suggest that there is a relationship between factors like diet and smoking to semen quality. Other studies have shown conflicting findings. A review of the literature on factors that may affect semen quality and male fertility is needed. 

Methods & findings

This study analyzed 96 published studies. These studies investigated a number of factors in relation to semen quality. Diet was analyzed as a factor. Other diet-related factors were nutrients, antioxidants (ATOs), caffeine and alcohol. Energy balance (EB) was also analyzed. EB was a term used to describe the amount of energy used and consumed. Body mass index (BMI; a measurement of body weight in relation to height) was an indicator of EB. 

Men that strongly adhered to a healthy diet had better sperm count and motility. A high intake of red and processed meat, fat and sweets were associated with poor semen quality. ATOs in the diet or supplements may be important for good semen quality. It is unclear which ATO or combination is most effective.

The effect of caffeine on semen quality is still unclear. Long-term alcohol use was associated with lower semen volume and changes in SPM. Increasing BMI was associated with poorer semen quality. 

The bottom line

The authors concluded that a healthy diet and weight are important for male fertility.

The fine print

Some important factors were not assessed in this study. These include physical activity, stress, sleep, and environmental pollution. More studies are needed.
