In a nutshell
This review looked at whether phosphodiesterase 5 (PDE5) inhibitors such as sildenafil (Viagra) can improve male fertility. It found that these medications improve sperm concentration, shape, and movement.
Some background
PDE5 inhibitors are the most common treatment for erectile dysfunction (ED). These medications increase the amount of nitric oxide (NO) in the arteries, which causes them to expand. Other examples of PDE5 inhibitors include tadalafil (Cialis) and vardenafil (Levitra).
Men with infertility are often prescribed PDE5 inhibitors. There is an overlap between ED and infertility. ED may worsen while trying to conceive, as sex becomes less spontaneous. Regular ejaculation can promote healthy sperm development. Also, NO may also play a role in sperm development. However, it is not clear whether PDE5 inhibitors can improve sperm quality.
Methods & findings
This review included nine studies with 1,211 men. All of the men were randomly assigned to a PDE5 inhibitor or a placebo (inactive pill). Four of the studies included men with infertility. Other studies included healthy volunteers or men with ED.
Men taking PDE5 inhibitors had a significant improvement in semen volume, sperm number, and sperm concentration. Also, significantly more sperm could move in a straight line (improved sperm motility) and had normal shapes.
The bottom line
This review found that regular use of PDE5 inhibitors led to better sperm quality.
The fine print
The studies recruited men from different backgrounds, which can make it more difficult to combine data. Some studies included men seeking infertility treatment, while other patients were seeking ED treatment. More controlled studies are needed.
