In a nutshell

This study investigated combined treatment with sacubitril/valsartan (Entresto) in patients over 55.

Some background

Hypertension or high blood pressure (BP) can be managed with a number of strategies. Medications used to treat hypertension are called anti-hypertensives (AHTs). Patients may take a combination of AHTs n order to manage BP. Sacubitril (SC) and valsartan (VAL) are a relatively new combination. SC/VAL is licensed for the treatment of heart failure. Some studies suggest it may also be useful in managing high BP. 

SC blocks an enzyme called neprilysin. This reduces the production of proteins that lower BP. VAL is a drug that blocks angiotensin receptors (ARB). This relaxes the muscle in the artery and reduces BP. It is unclear if SC/VAL is safe and effective in the treatment of hypertension.

Methods & findings

This study reviewed 5 clinical reports. These trials included 1513 patients over 55. Patients were treated with SC/VAL or an ARB alone. These ARBs included VAL and olmesartan (Benicar)

SC/VAL improved both readings (systolic and diastolic) of BP. The reduction in BP was significantly greater in SC/VAL-treated patients compared to either ARBs.

There was a 27% higher frequency of side effects reported in SC/VAL-treated patients. The most common side effects were the common cold, upper respiratory tract infection, and dizziness. Excess uric acid in the blood was another side effect. 

The bottom line

The authors concluded that SC/VAL was more effective than other ARBs for the treatment of hypertension in patients over the age of 55.

The fine print

The maximum duration of studies in the analysis was 1 year. The long-term effects of SC/VAL require further investigation.
