In a nutshell

This study examined the effects of B-TUEP, a new minimally invasive procedure to treat benign prostate hyperplasia (BPH). Researchers concluded that B-TUEP is a safe, durable, and effective alternative treatment option for BPH.

Some background

BPH can lead to bothersome urinary symptoms that impact quality of life. Surgery is often required to remove the enlarged prostate tissue. Removing enlarged prostate tissue decreases blockages and improves the flow of urine. However, first-line surgical procedures are associated with a number of possible complications. Techniques that involve lasers or heat or water vapor to destroy prostate cells are increasingly being applied as less invasive alternatives.

Bipolar button electrode transurethral adenoma enucleation (B-TUEP) is a more recently developed approach that uses ‘button’ electrodes on either side of the prostate. More trials are needed to determine the safety and effectiveness of B-TUEP.

Methods & findings

The aim of this study was to measure treatment outcomes of B-TUEP among men with BPH.

50 men with BPH were included in this study. All men were treated with B-TUEP. Questionnaires and physical examinations recorded treatment outcomes. Patients were followed for an average of 42 months.

Urinary flow rate was significantly increased 1 month after the procedure when compared to before treatment. All men maintained this improvement at 12, 24, and 36 months. Complete bladder emptying was also significantly more common after B-TUEP. 94 to 96% of men showed less than 50 ml of residual urine in their bladders after passing urine. Prostate size was significantly reduced after treatment (as measured by ultrasound at 12 months).

On average, self-reported urinary symptoms were significantly improved at 12, 24, and at 36 months. Patients reported enhanced quality of life at 12 months compared to before treatment. Quality of life scores continued to increase at 24 and at 36 months. From the 6th month after B-TUEP, over 50% of men were sexually active. Before treatment, 34% of men were sexually active.

The bottom line

Researchers concluded that B-TUEP is a safe, durable, and effective alternative treatment option for BPH.
